70year male with pancytopenia

CASE:
70 year male who is a farmer and resident of suryapet came with chief complaints of shortness of breath from 20 days
History of present illness:
The patient was apparently asymptomatic 20 days ago ,then he developed fever of high grade, intermittent not associated with chills and rigor and also complaints of pale hands.so he went to a hospital in suryapet ,where he got treated for fever and was given 4 units of blood and then referred here.
- The patient also has shortness of breath which is of grade 3 and increased on walking
-No complaints of vomiting, diarrhoea
Past history:
-He has no diabetes, hypertension, asthma, tuberculosis,epilepsy
-He got operated for an ulcer on left foot 4 year ago
Family history:
There is no significant family history
Personal history:
Appetite -normal
Diet- Mixed
Bowel and bladder habits-regular
Sleep - adequate
No addictions
General examination:
The patient is conscious , coherent and cooperative
Well oriented to time , place and person
Moderately built and moderately nourished
Pallor -present
Icterus- Absent
Cyanosis- Absent
Clubbing-Absent
Generalized lymphadenopathy-Absent
Pedal odema -seen on left foot and leg till shin and is of pitting type
Temperature:Afebrile
Pulse:90bpm
Blood pressure:110/80mmHg
Respiratory rate:14cpm

S1 and S2 heard
No added murmur
Respiratory system:
Bilateral air entry is present
Normal vesicular breath sounds heard
Abdomen :soft and non tender

Haemoglobin:3.7gm/dl(Decreased)
TLC:2,800cells(Decreased)
PCV: Reduced
RBC:1.12 million/cumm(Reduced)
Platelets: Reduced(10,000)
T3: Reduced
Sodium: Reduced
Provisional diagnosis:Pancytopenia
Treatment:
-Inj.Methylcobalamine 1000micro grams/IV/OD
-Inj.Iron Sucrose 1amp in 100ml NS/IV/OD
